How we handle patient enquiries

This notice explains how MeGMeT uses information submitted through the patient request form.

Information we collect

We collect the contact details, care pathway, short medical summary and medical documents that you choose to submit. Please avoid sending information that is not relevant to your request.

Why we use it

We use your information to review your enquiry, contact you, request clarification and coordinate a response with appropriate healthcare providers when you authorise us to do so.

Sharing

Medical information is shared only as needed for the care-coordination service you request. We do not use submitted medical reports for advertising.

Retention and your choices

We retain enquiries only for as long as reasonably needed for coordination, follow-up and applicable record-keeping. You may ask MeGMeT to correct your contact information or stop coordinating your enquiry by emailing info@megmet.org.
